Follow these steps to apply for Shade View Apartments or the Housing Choice Voucher Rental Assistance

Please review eligibility criteria for the programs and the to see if you qualify prior to submitting an online application.

IMPORTANT NOTICE: The online application is a FULL APPLICATION. It is not a pre-application. You must disclose all information, and failure to do so could result in the withdrawal of the application. Please make sure you select the correct waiting list. Shade View Apartments is public housing in Middleburg, PA. The Housing Choice Voucher Program is for help with your rent.

Step 1 : Before You Begin
Before starting your online application, please have available your social security card, names and dates of birth of all family members. Also, you will need income of all family members, all checking and savings accounts, stocks, bonds, real estate, online banking (CHIME, PayPal, CashApp, etc) and prior housing history. Make sure you have typed correctly all information.

You will need to create a PIN number. Please write down your pin number for future access.

Submit only ONE application. Duplicate forms will not be accepted.

Step 2 : Complete the Online Application
Click the button below to fill out the online application. NOTE : make sure to complete the following step (Step 3) in order to submit your full application.

IMPORTANT!!  You only have 14 days to complete your application from the date you start.  If you fail to submit the application within this time period, the system will automatically delete your incomplete application.

Step 3 : After Completing Your Online Application
Your application process is not complete until you submit the following forms to our office within fourteen (14) calendar days of submission of your online application.

They must be submitted with original signatures. Emailed or faxed copies will not be accepted. Failure to provide these documents within fourteen (14) days will result in the withdrawal of your application.

Maintaining Your Status on the Waiting List
If you are added to the waiting list, you will be notified by mail when your name comes to the top of the waiting list for an interview or if we are updating our files. If you do not respond to our notifications or your letter is returned undeliverable, your name will be removed from the waiting list. If you have a change of address, phone number, income or family size, you must notify us within ten (10) business days of the change. ALL CHANGES MUST BE IN WRITING. No changes will be taken over the phone. Please include your social security number on any correspondence.
